Lipectomy of Abdomen Thighs Transcription Sample Report

POSTOPERATIVE DIAGNOSIS:  Excess adipose tissue to the abdomen and bilateral thighs.

OPERATION PERFORMED:  Suction-assisted lipectomy of the abdomen and circumferential thighs.

COMPLICATIONS:  None.

ESTIMATED BLOOD LOSS:  Minimal.

SPECIMEN:  4000 mL of suction-assisted lipectomy product, discarded.

DESCRIPTION OF OPERATION:  The patient was taken to the operating room and placed in the supine position on the operating table. All appropriate monitoring equipment was attached. At this point, general anesthesia was uneventfully instituted, including with LMA placement. Ancef 1 g was given as prophylaxis. The entire operative site was prepped with Betadine in the usual manner, and sterile drapes were applied in the usual fashion.

A #15 blade was used to make openings above the umbilicus, in each groin, in the lateral thigh between the hip and trochanteric fat pads, the lateral thigh below the trochanteric fat pad, in the medial thigh, and in the medial leg, below the medial knee fat pad. Through these openings, tumescent fluid was injected, totaling 2000 mL, 1 mL of 1:100,000 epinephrine per liter of saline. Once white blanching was seen, suction was done using 4 mm short and long accelerator-tip cannula. We removed 250 mL from each side of the abdomen, 450 mL from the medial right thigh, and 400 mL from the medial left thigh, 200 mL from each anterior thigh, 600 mL from each lateral thigh, and then 200 mL from each posterior thigh. An additional 150 mL was removed from the abdomen as touch up and then an additional 450 mL from the thighs as touch up.

Reinspection was done multiple times, and we did make sure that the removal was as even as possible, leaving behind tissue that, visibly and by palpation, appeared to be equal. Suctioning was done through the multiple openings at different levels and in different directions, but trying to keep 1 cm fatty tissue attached to the overlying dermis. Very minimal bleeding and very minimal free fluid, less than 200 mL of free fluid in the suction canister.

The areas were cleansed and the incisions were closed with interrupted sutures of 5-0 Novafil. Additional cleansing was done and then dressings were applied using Band-Aids and antibiotic ointment to the incision sites and then portions of ABD were held in place with a thigh compression garment and then an abdominal binder. Needle and sponge counts were correct at the end of the case. The suction-assisted lipectomy of the abdomen and circumferential thighs was ended, and the patient was escorted to the recovery area, having tolerated the procedure and the anesthesia satisfactorily.
